~numbears: The main menu says reps are available, but doesn't say how to get to one. The GETHUMAN suggestion worked, but I would never have figured that out by myself. The person who picks up will be cranky because they are expecting you to ask them about a lost form, remember that they did not design the awful phone menu, be polite, and they will lighten up.
