~doofy: I filled out the application online and faxed the information they needed to the number provided. A month later, I tried to get into the site but my pin didn't work, so I called customer service. They had me at the wrong address, and had no record of my information being received by fax. They could not just correct my address, they had to send me a new application for the new address. If I didn't ABSOLUTELY need this phone, I would not go through this.
