~karen: Trying to find out what number to call for disability to follow up. I was notified Last Thursday my Disability has been approved. I have been on the phone for almost * hours trying to locate the right place to ask to have the form e-mailed to me. I was told last Thursday the gal was going to e-mail the form right then, but I did NOT receive it. Need to get this form.*I didn't get a phone number unfortunately.
